What a network is

A provider network is the group of doctors, facilities, and other providers that have contracts with a health plan. Using an in-network provider usually means lower out-of-pocket costs. A doctor’s location or the insurer’s name does not establish participation in a particular plan.

How to check

Start with the plan’s provider directory. Then confirm participation with the insurer or doctor’s office, using the exact plan name. Check the providers and facilities you expect to use.

Before changing coverage

Review the rules for care outside the network. Whether that care is covered, and what you might pay, depends on the plan and situation. Ask for clarification before making a coverage decision.

This guide is general education. Your own coverage decisions call for a review of your needs, eligibility, and available options.
